Community Treatment Order Coordinator (TPT 0.5 FTE)

Duties

may include preparation of paperwork, case conferences, liaising with treatment providers and community case managers, and providing support to family members and patients who are supported by Community Treatment Orders

Evaluate the effectiveness of the individualized CTO plan.

Develop and maintain necessary working relationships with hospitals and community service providers and programs who will assist with Community Treatments Orders and form the basis of the discharge plan into the community.

Develop and maintain a database on client demographics, referrals and outcome data.

Monitor and evaluate information regarding the usage of Community Treatment Orders, gaps in the system related to development of CTOs and the identification of program and service needs as identified by the data and stakeholders. Report these findings to appropriate authorities.

Participate with other regional CTO Coordinators in regular meetings.
