Summary The VA Northeast Ohio Healthcare System is recruiting for an Compact Act Registered Nurse. The Registered Nurse will function in the Patient Transfer Center (PTC) section of the Patient Care Administrative Service (PCAS). Responsibilities Duties of the Compact Act Registered Nurse include but are not limited to: Delivers fundamental knowledge-based care coordination to assigned clients. Demonstrates knowledge concerning the care and movement of patients throughout the facility and non-VA facilities. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Influences care outcomes by collaborating with members of the interdisciplinary team.
