In your position as a Registered Practical Nurse (RPN), you'll work closely with an interdisciplinary team to deliver high-quality, patient-focused care. Primarily working in the Way Home Unit, you'll support patients during their transition from hospital to home and may be called to assist in community care through the OSMH at Home program.
