Wye Valley NHS Trust and St Michael's Hospice, in the beautiful county of Herefordshire, are looking to appoint a new consultant post in Palliative medicine as a result of service expansion. The team provides medical leadership, direct patient care, training, teaching and opportunities for quality improvement and service development across WVT acute and community hospitals, Herefordshire and mid Powys community and St Michael's Hospice.
The post holder will act as Lead for COMPASS (Community Outreach Model for Palliative Assessment, Support and Step up/down) based within Hereford County Hospital and the Virtual Wards from Nelson House.
This post holder will work with an established specialist palliative care team including consultants, CNSs and End of Life Care Facilitators. This post will provide consults in acute receiving areas and review new and exist...
