Counsellor/Therapist 2 - Geriatric Outpatient Services

Job Description

Full‑time, permanent Counselor Therapist 2 on the Geriatric Mental Health Outreach team (GMHOT) at 80 Workman Way. Reporting to the Manager, the incumbent will provide direct clinical care for adults experiencing addiction or concurrent disorders who are living in long‑term care, and deliver addiction support and education to clients and long‑term‑care home staff.

• Conduct assessments of new clients from referrals.
• Develop treatment and care plans in collaboration with the physician.
• Engage family members, community supports, and long‑term‑care teams.
• Serve as a resource to the GMHOT team, offering guidance and support on addictions in geriatric populations.
• Work with clients from diverse backgrounds, applying a harm‑reduction, recovery‑oriented, and trauma‑informed approach.
