A rewarding opportunity for a passionate Dementia professional to join HammondCare on anPermanent Full-timebasis at our Woy Woy location.

Dementia Consultant, known within HammondCare Residential as a Dementia Advisor, focuses on improving the lives of individuals living with dementia by applying dementia-specific clinical knowledge and skills in a residential aged care setting.

As a key member, some of your responsibilities will include:

Implement HammondCare’s Philosophy of care and Case management framework.

On the ground support. Providing dementia specific expert advisory services within HammondCare services, through participation in regular clinical review meetings
