Diabetes Educator and Dietitian Role

Become a key part of the Sunnybrook Academic Family Health Team as a Clinical Dietitian focused on diabetes education. This full-time position, combining SUNDEC and SAFHT roles, serves a complex patient population.
As a Registered Dietitian, you will provide medical nutrition therapy for both Type 1 and Type 2 diabetes patients at the Sunnybrook Diabetes Education Centre and support primary care at the Sunnybrook Academic Family Health Team. Your role will emphasize individual and group education on diabetes management while collaborating with an inter-professional team to foster coordinated care and community outreach.
