Director Critical Time Transition Program

Job Summary

The Director of the Critical Time Transition Program for Children, Youth and Families (CTTP) provides leadership and direction to the CTTP team which is responsible for facilitating the safe transition of children/youth from a hospital setting to the community. The Director of CTTP will use a model that is trauma-informed and utilizes behavioral support principles, prioritizes family support and skill-building, and is built on a strengths-based, relationship-focused culture.

Essential Job Functions

+ Lead the design, development and implementation of the CTTP program, in alignment with state regulations, program guidelines and agency policy and practice, and in collaboration with hospital partners, external stakeholders and Hillside leadership. Collaborate with CTTP programs throughout the state to give input to OMH on the ongoing evolution of the CTTP program.
