Job Description

your responsibilities will include:
Being responsible for providing optimal discharge planning in line with the strategic direction, operational processes, budget and business direction of the hospital
Delivering seamless patient flow, effective bed utilisation and episode management as part of a dynamic team
Liaises with Unit Managers, nursing staff, allied health Professionals and Medical Practitioners to coordinate patient discharge plans
Liaise with external service providers for patients with ongoing care needs post discharge
Supports and facilitates the patient and family by providing appropriate information and options regarding health and nursing care
Early screening of inpatients with complex care needs to facilitate timely discharge
Ensures information relating to patient discharge plans is documented and accessible by all clinical staff
