About the Role
The District Nurse is responsible and accountable for assessing, planning and implementation of programmes of care. These will be developed in partnership with patients, family, whanau/carers and significant others and will be evidence based taking into consideration lifestyle, race, gender and cultural background and factors which impact on physical, mental or emotional health. The District Nurse works in partnership with patients and their families/whanau and collaborates with the multidisciplinary team in both the primary and secondary health settings and recognises and supports the delivery of Health New Zealand Te Whatu Ora Lakes vision and demonstrates the values in the workplace.
