An exciting opportunity has arisen for a motivated, enthusiastic qualified District Nurse to become the Team leader of the Lightwood Greenhill Team within the Community Nursing service.
The post holder will lead, manage and supervise a Community Nursing Team, facilitating research and evidence-based practice. Lead the assessment process and maintain high quality effective nursing care in the community as part of the wider integrated care team. Other duties will be to ensure service delivery is in line with the Core Offer and the Standard Operating Procedures of the Community Nursing Service
This large dynamic team is part of the wider Integrated Community Care structure and works collaboratively in order to deliver high quality, individualised patient care at home.
