Job Description: Responsibilities: Provide high-quality nursing care and emotional support to patients, ensuring their comfort, safety, and well-being. Assess patients' clinical conditions, monitor vital signs, and administer prescribed medications and treatments accurately. Develop, plan, and implement individualized care plans in collaboration with the healthcare team. Serve home care clients through hands-on case management and conduct hospital or home visits for client assessments. Support the home care team with nursing cases and take up respite assignments when required. Advise and guide care staff in managing medical issues or emergencies. Communicate effectively with patients' families, providing timely updates on care and progress. Collaborate with colleagues and external community partners to coordinate and manage client care needs.
