Job Scope

Assigned Service – Home Palliative Care

• Manage referrals and alert clinical team on new cases

• Ensure acceptance/rejection within stipulated timelines

• Update patient data in finance & operational systems

• Handle billing, fee collection, Palliative subvention, MediSave & MediFund claims

• Maintain service utilisation records for reporting

• Update HCS administrative, financial & operational manuals

• Support MOH, AIC and regulatory audits

• Communicate with patients/families, understand needs and escalate to nurse-in-charge when required

• Coordinate with team members for effective problem-solving
