Experience Community Health Worker

We are looking for experience for Health Practitioner/Health Coach/Community Health Coordinator to support our US team.

Location: Onsite/Residing Philippines
Employment Type: Full-time/Independent contractor

Duration: Long term

Responsibilities:

· Act as a primary non-clinical point of contact for patients

· Conduct wellness check-ins, follow-ups, and patient engagement activities

· Provide basic health education, lifestyle guidance, and wellness coaching (non-diagnostic)

· Perform community or field-based coordination, including local follow-ups and support as needed

· Assist patients with scheduling, referrals, and navigating health or wellness services

· Collect patient information and escalate clinical concerns to the Registered Nurse

· Maintain accurate documentation and follow escalation and compliance protocols
