Family Care Partnership Care Management Manager

Humana/iCare is seeking a Care Management Manager to join the team in the Family Care Partnership (FCP) program. You will report to the Associate Director, Care Management Support. You will oversee a team of nurses and human service professionals working in the field who are responsible for care management services through the FCP program. Develop your leadership skills all while doing your part to improve the lives of others.

**Responsibilities:**

+ Screen, interview, orient and establish dynamic and successful associates.
+ Manage and evaluate the performance of associates assigned to their teams.
+ Promote a collaborative team environment and positive work culture.
+ Provide leadership and feedback to encourage professional development and associate retention.
+ Convene team meetings to the Interdisciplinary Team (IDT) to provide case staffings and share information.
