Family Help Lead Practitioner – Edge of Care Team

• Dedicated to keeping children and young people safely within their families and communities.
• Provide trauma‑informed, restorative, and strengths‑based support and deliver evidence‑based interventions to families at risk of breakdown.
• Work intensively with families to reduce risks, strengthen protective factors, and keep children safely at home.
• Respond promptly to escalating situations, implementing safety plans and de‑escalation strategies to prevent family breakdown.
• Develop and implement tailored family plans in partnership with parents, children, and multi‑agency professionals.
• Collaborate with Social Work and other agencies to ensure a consistent approach within the child's plan.
• Contribute to the development of a dynamic approach to supporting young people and their families.
