Family Hub Practitioner 11-19

Role Responsibility

In this role, you will operate within the Thanet Family Hub team as a Family Hub Practitioner supporting young people aged 11-19 or 25 with SEND (Special Educational Needs and Disabilities). As well as providing groups and activities within the Family Hub you will also deliver activities and services within outreach settings, such as schools, and 1:1 support within the home for young people and their families.

You will be responsible for supporting improvements in outcomes for children and young people through the delivery of group and 1:1 interventions and opportunities across Family Hub sites, family homes, other community settings and outreach locations.

The post holder will be expected to operate a Whole Family approach in their practice and work around the times of the family. As such, this post will be expected to work flexibly including, on occasions, at evening and weekends, and will be co-ordinated by Management as requ...
