Job Summary: Intention Healthcare is seeking a compassionate and skilled Nurse Practitioner (NP) to provide high-quality, patient-centered care through home visits in the Greater Boston region of Massachusetts. This role focuses on individuals with chronic illnesses, mobility challenges, or limited access to healthcare facilities. Responsibilities include:

Patient Evaluations: Conduct comprehensive health assessments during home visits, including physical exams, medication reconciliation, and review of medical histories.
