Family Support Worker

Love Ones Care is currently seeking a compassionate and dedicated Family Support Worker to join our growing team. If you are passionate about empowering families and supporting individuals to achieve positive outcomes, we would love to hear from you.

Key Responsibilities:

1. Identifies families who require additional support and provides internal/external referrals
2. Ensures families can access appropriate services, and encourages families and individuals with particular needs to participate in informal discussion, counselling or support groups
3. Assists and collaborates with Social and Welfare Workers to develop holistic plans by establishing family assessments, safety plans, goals and interventions to ensure safety and wellbeing, and empower families towards positive change
