Job Description
Support the well-being of 2SLGBTQ youth as a full-time Case Manager at Sherbourne Health in Toronto, Ontario. Collaborate with young adults aged 14-29 by providing tailored case management that encourages personal achievements.
As a junior member of the Community Programs team, your role will involve conducting intake assessments and facilitating access to necessary health services. You’ll also work collaboratively with various partners to develop Smart plans. Strong crisis management skills and the ability to engage positively with service users will be essential for success.
Key Responsibilities:
• Conduct intake assessments and set goals for service users
• Facilitate communication among service users and partners
• Provide brief counseling and urgent support as required
• Organize outreach workshops and group activities
