Future Care Planning Resource Centre (FCPRC) - Manager

Overall Job Responsibility
The Manager Supports The FCPRC’s Efforts To Develop The Ecosystem Of Support For Future Care Planning (“FCP”). These Efforts Include, But Are Not Limited To, The Following

• Resource development for FCP;
• Capability building of sector professionals to deliver FCP to their clientele;
• Support caregivers in FCP through triaging and routing of cases to relevant sector professionals;
• Community engagement, public education and targeted outreach on FCP amongst caregivers and key stakeholders;
• Building collaborations with stakeholders/enablers within SG Enable and external agencies from relevant sectors to facilitate the FCP process.
  

The Manager also develops and implements other initiatives to fulfill the role of the FCPRC and achieve its goals.
