Geriatric Nurse Practitioner

**Job Summary and Responsibilities**

Dignity Health Medical Group - Arizona (https://www.dignityhealth.org/dhmg-arizona) is seeking a full-time Geriatric Nurse Practitioner to join our busy department in Prescott, Arizona

+ EMR: Cerner
+ This position will be Alzheimer’s and Dementia Care focus.
+ Project volume: 250 patients
+ Office Patients: Patients are seen in the office with some off-site home visits in the surrounding area
+ Typical work week (hours worked; hospital vs. clinic) - option to work 4 or 5 days per week. Working in clinic setting, as well as, off site visits throughout the community to accommodate the patient’s needs. Typical patient visit load of 2 new consults a day and f/u visit

**Job Requirements**

+ Current Board Certification
+ Valid medical license
+ Valid DEA certificate
+ 2-5 years experience in geriatrics with focus on cognitive diseases preferred
