Geriatrics Nurse Practitioner

Deaconess is seeking a full-time ACNP, FNP, or PA to join the Geriatric Workforce Enhancement Program. This is a fully grant funded position to serve as the Clinical Program and Evaluation Specialist for our Geriatric Workforce Enhancement Program. The GWEP is a partnership between USI, Deaconess, and AAA sites to improve the care for older adults in our community. Deaconess role is largely focused on geriatric care in the primary care setting. The current 5-year grant program is geared towards clinical practice, social determinants of health and health inequities and geriatric education and training needs of health workers which will allow us to increase the knowledge of PCP providers and embed the 4 Ms workflows into their practice to strengthen the care for our geriatric population. Typical working hours will be from 8:00 am - 4:30 pm.
