HCC Risk Adjustment Coder (Epic EMR) | Permanent WFH

We’re looking for a detail-obsessed HCC Risk Adjustment Coder with strong Epic EMR experience to help ensure accurate diagnosis capture and compliant risk adjustment coding. This role is all about reviewing provider documentation, assigning the right ICD-10-CM codes , and making sure everything aligns with CMS HCC guidelines —because clean data = clean RAF scores.

Key Responsibilities

• Review medical records in Epic EMR to identify and accurately code HCC-relevant diagnoses
• Assign compliant ICD-10-CM codes following CMS HCC risk adjustment guidelines
• Ensure diagnoses meet MEAT criteria (Monitored, Evaluated, Assessed, Treated)
• Identify and close care gaps related to chronic condition recapture
• Perform retrospective and/or concurrent HCC chart reviews
• Collaborate with providers and clinical teams to clarify documentation when needed
