Health and Wellbeing Co-ordinator

About the role

We are seeking a motivated and compassionate Health and Wellbeing Coordinator to join our team. This role is central to delivering our core aim: supporting individuals who are at risk of homelessness, currently experiencing homelessness, or who have previously experienced homelessness, to sustain a tenancy and improve their wellbeing.

This position is subject to a satisfactory PVG check.

Responsibilities

You will work closely with individuals to build trust, promote independence, and support long‑term tenancy sustainment through flexible, open‑ended support.

Qualifications

• Deliver high‑quality, person‑led support to individuals with complex needs in line with Housing First principles.
• Work collaboratively with individuals, their support networks, and the GCHSCP Housing First Assessment Team to determine access and ongoing support needs.
