Health Educator - Ambulatory Care Management

Description

The Ambulatory Care Management (ACM) Health Educator assumes responsibility and accountability for conducting Department Orientation for new staff, ongoing education, skills and competencies for existing staff including but not limited to Annual Competencies and Skills. The Educator will report to the Clinical Manager, Education. The Health Educator will develop curriculum, orientations, competencies, as well as audit tools approved by ACM Leadership. In addition, The Health Educator assists in Quality Assessments and other Administrative Duties as assigned by ACM Leadership. The Health Educator - a seasoned Comprehensive Care Coordinator, Community Health Worker or Clinical Social Worker - is responsible for assisting with clinical and/or administrative management, training and coordination within the scope of their practice across ACM.
