Summary
We are looking for a non-registered health professional to provide patient support, coordination, and community-based engagement. This role focuses on non-clinical care, including follow-ups, education, and on-the-ground coordination, while working under the guidance of a Registered Nurse or clinical team.
This role is ideal for health coaches, nutritionists, wellness practitioners, allied health workers, or community health coordinators.
