We are looking for experience for Health Practitioner/Health Coach/Community Health Coordinator to support our US team. 
Location: Remote/Residing Philippines
Employment Type: Full-time/Independent contractor
Duration: Long term
Shift: US Day shift
Responsibilities: Act as a primary non-clinical point of contact  for patients 
Conduct wellness check-ins, follow-ups, and patient engagement activities 
Provide basic health education, lifestyle guidance, and wellness coaching  (non-diagnostic) 
Perform community or field-based coordination , including local follow-ups and support as needed 
Assist patients with scheduling, referrals, and navigating health or wellness services 
Collect patient information and escalate clinical concerns to the Registered Nurse  
Maintain accurate documentation and follow escalation and compliance protocols
