Purpose

This Request for Proposal (RFP) is issued by HealthPRO Procurement Services Inc. (HealthPRO Canada) in order to develop national contracts for I.D. Bands , which meet the specific clinical and business needs of our committed membership and to qualify products/services (collectively Products) and suppliers/distributors (collectively Suppliers) for inclusion in HealthPRO Canadas Qualification Supplier Roster (QSR). Suppliers selected for inclusion will enter into a Qualified Supplier Roster Agreement (Roster Agreement).
Suppliers responding to this RFP must be a single legal entity that, if selected, intends to enter into a contract award, and to be included in HealthPRO Canadas QSR. If the response is being submitted jointly by two or more separate entities, the response must identify only one of those entities as the Supplier....
