+ Complete physical (within 1 year)

+ Drug Test (within 1 year)

+ PPD or Quantiferon (within 1 year)

+ Measles/Rubella/Rubeolla immunization records

+ 2 reference letters EVR's (Personal and/or Professional)

+ Valid ID and Social Security Card
