Healthcare Organization (HCO)  
 A contracted vendor for the New York Independent Assessor Program (NYIAP)  in partnership with the New York State Department of Health . Is responsible for conducting conflict free Medicaid assessments  for individuals seeking personal care services  and community based long term services and supports . 
 Nurse Assessors  complete the Community Health Assessment (CHA) .
