Home Health Admissions Coordinator is expected to support the mission, vision, and values of Olathe Medical Center, Inc. The major role of this position is to work collaboratively with all referral sources for the agency in providing transfer of patient care from any setting to the home-based services the agency provides.

Responsibilities And Essential Job Functions

• Effectively coordinates patient admission process for Home Health department according to Home Care regulatory standards.
• Coordinates scheduling for clinical field staff according to professional scope of practice and Home Care regulatory standards. Consults Clinical Supervisors when indicated for assignment changes and productivity adherence.
