Job Description

                    Hospice Licensed Practical Nurse  
Salary:  $31.25-$34.13 BOE 
Schedule:  Fri/Sat/Sun | 7 AM-7 PM 
Location:  Hamilton County, Ohio, and surrounding area 
Requirements of the Hospice LPN role:  
Licensed Practical Nursing Degree  
A minimum of 1 year of Clinical experience/Skilled care  
Hospice experience preferred  
Valid Drivers License and Auto insurance Coverage  
Reliable transportation: Willing to travel  
About the Hospice LPN ro le:  Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients.  
 Provide on-call services after hours, and complete scheduled skilled visits.  
 Assist Registered nurses in carrying out the plan of care.
