Position Overview:
A mission-driven, interdisciplinary care program is seeking a Board-Certified/Board-Eligible MD or DO with a background in Internal Medicine, Geriatrics, or Family Medicine to join its Skilled Nursing Facility (SNF) Transitions Team, with a strong focus on geriatric and palliative care. This is a unique opportunity for a physician with hospitalist or SNF experience seeking a rewarding role with excellent work-life balance.

Key Responsibilities:
Provide compassionate, high-quality subacute and palliative care for medically complex older adults in contracted SNFs

Manage a census of patients, with 5 average admissions/discharges per week

Conduct bedside rounds 2 4 times weekly with flexible hybrid work options
