Housing Stabilization Case Manager $2500 retention bonus

This position is eligible for a $2500 retention bonus.

Job Summary:

The Providence Center Housing Stabilization Case Manager provides housing case management support for the Providence Center’s housing department. Provide Housing stabilization services to participants. Complete filing, documentation preparation and use/development of spreadsheets in Excel part of daily work. Data entry in EPIC and coordinate referrals for supportive services.

Duties and Responsibilities:

Perform a variety of duties to support consumers and to staff in the department.

Work in collaboration with supervisor on scheduling interactions with recipients to meet monthly target measures.

Collaborate with staff to open Housing Episodes in Epic to ensure participants have a medical record number in order to complete documentation of services provided.

Respond to incoming telephone calls from internal/outside agencies, insurance companies...
