Summary


Provides professional patient care and practices physical therapy by implementing and documenting treatment following evaluation by the physical therapist. Treats patients in accordance with the order from the physician and the plan of care established by the physical therapist and provides treatment through the use of a variety of therapeutic procedures and assistive devices intended to maximize functional potential in all areas of the patient's life. Implements an individualized treatment plan based on the therapist's evaluation and plan of care to address patient needs. Collaborates with Physical Therapist to identify updates and revisions to treatment plans, as needed, throughout the treatment process. Establishes and instructs patients, families, and other caregivers in exercises and therapeutic home programs to be continued following discharge.
