                    HousingFirstisanalternativetoasystemofemergencyshelter/transitionalhousingprogressions. Ratherthanmovinghomelessindividualsthroughdifferentlevelsofhousing,HousingFirstmoves thehomelessindividualorhouseholdimmediatelyfromthestreetsorhomelesssheltersintotheir ownapartments.Clientswilllikelypresentwithcomplexandcompoundinghealth-relatedconcerns.

 Typicaldemographicswillhave a seriousmentalillnessthatisoftenaccompaniedbysubstanceabuse, historyofinvolvementinthecriminaljusticesystemandchronicphysicalillness.Amulti-disciplinary andteamprovidesaservicethatisbasedonintensivecasemanagementandassertiveoutreach. Requirements Responsibilities Provideinvivofollow-upsupport,assistance,andbasicskilltrainingtohigh-riskadultmen,womenandfamiliesinordertoestablishandmaintainanindependentandstablelivingthe situationwithinthecommunitythattheyhavebeenrecentlyhousedin.
