Intensive Case Manager

UnderthesupervisionoftheHousingTeamLead,theIntensiveCaseManagerworksasapartofateamprovidinghighqualitycasemanagementservicestoindividualseligibleforandtransitioninginto housing. Theintensivecasemanagershallserveasthecentralpointofcontactforindividuals,astheynavigatethetransitionfrom personexperiencinghomelessnesstotenantlivingwithinthecommunity.
