Summary The Intermittent Licensed Vocational Nurse (LVN) provides patient-centered, team-based nursing care to Veterans at the Santa Rosa South VA Outpatient Clinic. The LVN delivers high-quality, coordinated care focused on prevention, chronic disease management, health promotion, and continuity of care. The LVN serves as a key member of the interdisciplinary PACT team, supporting timely access to care, effective communication, and population health management. Responsibilities **This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until July 24, 2026. The initial cut-off date for referral of eligible applications will be June 9, 2026 with subsequent cut-off dates every 2 weeks.
