 Schedule 
 Clinic Monday - Friday 8a-5p 
No call
 Clinical Details 
 75% Adult, 25% Adult Geriatric 
 18-20 patients per day 
 No Hospital coverage 
 Chronic disease management, Acute care, hospital follow up, AWV, inbox management 
 Requirements 
 Board Certified IM 
 Active AZ License or IMLC 
 DEA 
 BLS, ACLS 
 EMR 
 Cerner
