What’s the role?
As the ITC Program Care Coordinator, you will support individuals and families living with complex chronic conditions by coordinating care across local and specialist services. You will work closely with the multidisciplinary team to develop and implement personalised care plans, strengthen referral pathways, and improve access to services.
The full-time position will be Darwin based, with a fly-in-fly-out (FIFO) schedule of 2 weeks per month based in Maningrida (Monday to Friday) and the other 2 weeks per month based in Darwin. Othe...
