GENERAL RESPONSIBILITIES:

The Physical Therapist, working as part of a multidisciplinary team, provides direct patient care to evaluate and treat injury, disease and other conditions of health. Responsible for planning and administering physical therapy treatment plans designed to alleviate pain, restore function, prevent disabilities, and promote overall fitness and health of patients. This position functions with a high degree of clinical autonomy, providing physical therapy services independently within the health center setting while collaborating with medical, behavioral health, and care management team members as needed.
