Bassano Health Centre is an acute and continuing care facility. Core programs at the Bassano Health Centre include 24-hour emergency, medicine, ambulatory care services, primary care clinic services, and community health. Bassano Health Centre has 4 acute care and 8 continuing care beds.As a member of the CoAct team, you will provide a variety of nursing services to patients and families while taking necessary steps to ensure their safety and well-being. The LPN provides safe and effective nursing care according to AHS policies and Continuing Care Health Service Standards. Working within the full scope of practice, the LPN observes, monitors, reports, and documents changes in the patient's physical, mental, and emotional state and assists the patient with activities of daily living. The LPN is responsible to collaborate as a member of the interdisciplinary health care team, delivering consistent quality care to meet the needs of the patient.
