Overview

Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home. 
Team 10 Covers: Babylon, Bayshore, Brightwater, Deer Park, Gilgo Beach, North Babylon, Oak Beach, West Babylon, West Islip, Wheatley Heights, Wyandanch
5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Additional compensation for visits over your caseload

Job Details

Our Home Care LPN’s:
Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
Measure vital signs, Administer IVs, change dressings, clean wounds as needed
