Description

First Choice Hospice (Jet Health, Inc.)is looking for PRN/PTLicensed Practical Nurses (LPNs / LVNs)to join our dedicated team to provide community-based care for our patients currently receiving hospice services in theTreasure Valley area.


The primary function of theLPNis to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.
