Licensed Practical Nurse – Home Health

Shift: 8 hours

Contract Weeks: 14 Guaranteed Hours: 40

Requirements:

• Current Nursing License in the practicing state
• Valid Driver's License
• Current CPR Certification
• Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice
• Ability to use electronic medical charting and time-keeping software

Roles & Responsibilities:

• Administer skilled nursing care to patients in their residence as prescribed by a physician.
• Collaborate with the interdisciplinary team to coordinate patient care and ensure compliance with standards.
• Educate patients and families on self-care techniques and assess their responses.
