Job Responsibilities:

• Provide follow-up skilled nursing visits for home health patients.


• Report patient care/condition/progress to patient's physician on a continuous basis.
• Prepare visit/shift reports and updates/summarizes patient records Assist with prescribed medications for patients Take vital signs as prescribed.
• May record and restock medication inventories.
• Assure that established infection control practices are maintained.
• Report all hazardous conditions and equipment to the supervisor.
• Keep the supervisor informed of supply needs.
