Licensed Practical Nurse

JOB SUMMARY

As a member of the interdisciplinary EMP team, the Licensed Practical Nurse is responsible for providing quality patient care within the scope of professional LPN practice to patients and families throughout their life span.

The incumbent will be part of the EMP Enhanced Care Coordination in Schools (CCS) program and will provide support to the team in their region.
The goal of this team is to ensure enhanced clinical support to students with complex needs in public schools by providing care coordination and supporting Education and Early Childhood Development (EECD) and integrated care teams in place.
