**Education Requirements**

+ High School Diploma/G.E.D. (required)

**Minimum Experience Requirements**

+ Two years of recent LPN experience (preferred)

**License/Certifications Requirements**

+ Licensed Practical Nurse (required)
+ Basic Life Support (required)
+ IV Therapy (Required upon hire or within three month of hire/transfer)
