Schedule: Monday-Friday, 8am to 5pm

Pay: $26.66 to $33.28 per year

Job Summary

The Licensed Practical Nurse (LPN) is a member of the PACE interdisciplinary team; provides nursing care to the participant and assists the Registered Nurse with the implementation of the Plan of Care. The LPN provides care for the participant in a manner that promotes an interdisciplinary team approach, respects participant's goals for care, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects th...
